Provide a way to discover the available reasoning efforts per model and way to set the desired model + reasoning effort via ACP

This should be resolved as of v0.0.421. ACP clients can discover available reasoning effort levels per model and set the desired effort via session config options. If you believe this isn't resolved, please feel free to reopen this issue.
