Place home view avatar/header bar at the top of the Graph view

Description

• Move the avatar bar (with account and integrations info) from the top of the home view to the top of the Graph view. This keeps account and integrations info always available when the Graph view becomes the main view of the GitLens panel
  • Ensure responsive behavior for different layouts & sizing
