Adds rules for asciidoctor output that main.css did not previously
style: block titles, listing/literal spacing, sidebar callouts,
description lists (dlist/hdlist1/dd indent), horizontal hdlist tables,
data tables (tableblock + frame-ends/grid-all/halign/valign + caption),
and decimal numbering inside .olist (counteracting the global
li{list-style:circle}).
