MPC-HC (media player) is playing a file in a secondary, monitor. When that app is active, alt-tabbing won't show MPC's icon. A side effect is that the order of alt-tabbing is messed up because now the first icon showing is the next app, since MPC isn't there, so the alt-tab will point to the next app after that so in effect the continuous alt-tab (one time) will cycle through the first 3 apps (MPC included).
This doesn't happen with a few other apps I sent to the secondary monitor. No other media player program to try, right now.
Also, while in the MPC subject, the icon that shows in alt-tab, isn't the correct one. Instead of the default MPC icon, it shows the "LAV Audio Decoder". While MPC is playing, it loads LAV Audio Decoder, LAV Video Decoder and LAV Splitter, so maybe it picks the icon from one of the three.
