docs(coc): route conduct reports to OU address (#115)

Replace the three CODE_OF_CONDUCT.md occurrences of
`jonathan.jewell@gmail.com` with `j.d.a.jewell@open.ac.uk`. The OU
address is the preferred public-facing contact for affinescript conduct
matters; the gmail address from #114 was the best I had at the time but
not the right channel for this purpose.
