refactor: RSR-compliant root cleanup — move design docs, delete stale snapshots

Move 6 design docs to docs/design/ and docs/. Delete 14 stale status
snapshots, executed plans, and duplicate files. Rename AI.a2ml to
0-AI-MANIFEST.a2ml. Add CHANGELOG.md and ROADMAP.adoc.
