Ensure all citations use an include

For consistency there shouldn't be any manual citations.

I just tried this, and it works surprisingly well:

git grep '[A-Z][a-z]*, [A-Z]\.'

As part of this: there are still two pages that need their citations converted to DOIs:

• plugins/spim-registration/method
• software/fiji/publications

Another thing that's part of this is to decide what to do about citations that can't use a DOI. Should we reinstate the more complex field-by-field declaration? Or just hardcode these without an include? I'm not sure.
