chore: manually update index.html

The previous fix incorrectly inlined the CCIP code as a separate Parcel
runtime, causing "Cannot find module 'cMvw3'" errors.

This fix simply removes the broken <script src=/index.js> reference
without inlining anything, keeping just the main bundle.
