docs(plans): add Status frontmatter to 12 landed plans

PR #651 flipped README badges; this follow-up adds a Status line to each of the 12 corresponding plan files so the in-file frontmatter matches the master index. Each line cites the landing commit + a one-sentence verification pointer (key file path or component name) so future readers don't have to re-grep.
