docs(changelog): audit Unreleased for reader-friendliness

Phase-2 organization-and-concision pass on the [Unreleased] section.
Technical content unchanged; structure, calibration against v4.0.0, and
skim-affordances tightened.

Summary block: fixed the "Six linkers (...8 names listed...)" miscount;
parallelized the four fold types into a clean comma-list; dropped the
Wave 6 PR 4 anecdote.

Concept-axis declarations: trimmed implementation noise (accessor-mirror
notes, default-value redundancy, JSON-shape minutia).

Audit-findings docs: collapsed the 12-entry per-cluster mapping into a
single summary sentence; readers who want the mapping go to the audit
docs themselves.

Methodology hardening / Audit / regression-guard infra / Hooks & DX:
trimmed "Trigger:" / "Closes a read-side gap" process narrative.

Concept-axis migrations (the largest restructure): replaced wave-numbered
top-level structure with phase + axis grouping (matches v4.0.0's "Phase
4b / Phase 3 / Earlier phases" calibration). Phase 3 has 8 sub-bullets,
one per family fold, grouped by axis. Dropped the gRPC/MQ assignment-form
correction, phoenix_ipc.py f-string fold, and "withdrawn ADR" sub-bullets
(commit-message material). Folded the standalone Pattern.framework_role
bullet into the framework-role fold sub-bullet. Compressed the
multi-paragraph Wave 6 sub-bullet to a single sub-bullet. Tightened the
verdict-correctness re-audit bullet.

Inheritance linker / Bakeoff infrastructure / Cross-language hygiene:
light tightening (dropped prometheus regression anecdote and forward-
narrative framings).

Framework-dispatch correctness: reframed the "Six linkers" header to
honestly label all 7 things; dropped test-count tails (calibration with
v4.0.0/v3.0.0 doesn't carry test counts).

Result: ~91 → ~84 lines, comparable to v4.0.0's 76 lines. Tracker
[Unreleased] left alone — single bullet, calibrated against v0.5.0.
