Create a template for filing issues against the CP/CPS. The template requires the reporter to specify a version and a section, so we can better track which issues are still relevant.

Create a template for filing issues against the CP/CPS. The template requires the reporter to specify a version and a section, so we can better track which issues are still relevant. Users within the letsencrypt org will still be able to bypass the template and file a blank issue if they so choose.
