I don't see what I should be seeing.
I have 3 questions but "entry" only shows up 2x & they are both for the field "name"

Email
Name
Phone number.
How I set this up:
Email: SETTINGS --> RESPONSES -> COLLECT EMAIL ADDRESSES -> RESPONDER INPUT
Name: SHORT ANSWER -> TEXT -> CONTAINS ^[a-zA-Z]{4,20}$
Phone number: SHORT ANSWER -> REGULAR EXPRESSION -> MATCHES [0-9]{10}
