Three new incidents sourced from a NotebookLM-grounded Pipeline F analysis
covering April–June 2026 session data (34 sources, 8 queries):
#39 — Fabricated verification record propagated through handoff/memory/auto-memory.
Lesson: cross-artifact prose agreement ≠ evidence; SoR audit trail is.
#40 — Mechanism DCs passed while goal metric (token count) was unchanged.
Lesson: first DC must measure the goal metric, not the build mechanism.
#41 — Longitudinal hook vs text-rule data: violations dropped 31→13 with hooks,
climbed back to 27 as text-only rules were added. Hook-backed rules had
zero recurring violations throughout.
