Add non-denominational (financially-speaking) payment options to join.html

Currently there are automated scripts to pay specified amounts via PayPal and Bitcoin. We need unspecified options for courses.

I tried adding <option value="Enjigo Course">Course Fees</option> for PayPal, but that errors out.
