Change SSE transport to streaming transport for MCP samples and docs

The new release of the MCP protocol and modelcontextprotocol NuGet package have deprecated SSE transport. Pull request #212 updated the packages and fixed the build breaks, but there are some remaining issues:

• Rename projects and namespaces to remove the SSE wording.
• Review and update documentation for wording changes as required. File separate issues if there are major changes.
