fix: overwriting of listchanged

Change description

• I removed the automatic capability registration entirely which was overwriting the listChaged to true .
• Now user has to explicitly pass this info

Motivation and Context

• This fix addresses McpServer overwrites server tool capabilities when a tool is registered #1488 where McpServer was unconditionally calling registerCapabilities({tools{listChanged:true} })inside setToolRequestHandlers(), overwriting any user-provided capability settings

How Has This Been Tested?

• I have only ran lint:all test:all not tested in a real application . but open to any suggestion .

• I dont think any breaking changes . No breaking changes require . They just have to explicitly pass the tool.listChanged to a bool value.
