Add experimental MCP daemon for live Hunk sessions

Summary

• add an experimental hunk mcp serve daemon on loopback for agent-to-diff communication
• register live Hunk TUI sessions over a websocket broker and expose MCP tools for session discovery and inline comments
• map incoming comment tool calls into the existing inline note UI, with docs and tests for the new flow

Testing

• bun run typecheck

• manual tmux smoke test using bun run src/main.tsx -- mcp serve and bun run src/main.tsx -- diff ... from the repo source
