feat: add comprehensive integration tests

Add 7 new integration tests covering:
- Operation filtering (read/delete operations)
- Path-based rules with startsWith
- Combined conditions with AND operator
- Multiple rule groups routing
- Non-matching rules verification
- Auth failures with policy denials
- AppRole authentication flow

Total integration tests: 12
