refactor: reduce cognitive complexity in fragments.py and graph.py

Extract helper functions to reduce SonarCloud cognitive complexity:
- fragments.py: extract gap fragment helpers, bracket balance helpers
- graph.py: extract import resolution, config key patterns, lexical edge helpers
