fix: allowances

• For parity w/ legacy Inst Dash: if an instructor grants an allowance for a user for an exam that the user ALREADY has an allowance for, overwrite the previous allowance.
• For the Edit Allowance modal: The Exam Type is blank. It is correctly inactive, but should have the correct allowance type shown.

• If a student has already attempted an exam, if an instructor clicks 'edit' or 'delete' do not allow the action. Instead show a dialog that says "Cannot [edit/delete] allowance: {username} has already attempted the exam"

Issue 2 is in progress, we catch that one too 😅 openedx/openedx-platform#38458
