Trigger react-on-comment workflow on issues opened

Add `issues: types: [opened]` event trigger so the workflow also
reacts to newly opened issues that mention the trigger phrase.

Add `github.event.issue.body` as a fallback for COMMENT_BODY so
the issue description is checked when no comment or review body
is available.
