poly_tomont ppc64le: restore full unrolled body in generated copy

Fix a bad hand-edit of the generated mlkem/ file that dropped one of
the four unrolled iterations. Restores the byte-for-byte simpasm output;
only the header attribution line is removed vs the reviewed version.
