docs(apps): rewrite url-schemes guide for new portal

- switch stream URLs to product-namespaced /vesting/stream and /payments/stream
- document /details, /history, and campaign /manage sub-routes
- replace query-only search with path-based /vesting/search/{chain}/{token?} and /payments/search/{token?}
- add new query params: f, so, sd, p, o, n, cc
- move airdrops from /airstream to /airdrops/campaign and /airdrops/search
- note query-param address case handling and add legacy → new mapping table
