chore: add .git-blame-ignore-revs for oxfmt reformat

GitHub's blame view skips commits listed here automatically. Local git
blame users can opt in via:
  git config blame.ignoreRevsFile .git-blame-ignore-revs

Seeds with the oxfmt template reformat commit from this branch.
