error reporting / logging / telemetry on apps & services

When things go wrong with one of Registry's many hosted services (goldsky, goldsky's hosted postgres database, fly.io api process, cloudflare-worker-powered frontend) I want us to have good info about what went wrong, and probably (for now) a message sent to us in Discord. For the time being, I would love to get a message every single time our API service throws a 500 error.

• API & Goldsky repo: https://github.com/stellar-registry/indexer/ — the API service here seems like the main culprit for intermittent 404s on the rgstry.xyz frontend. Better error reporting & telemetry on is the main deliverable for this issue.
