docs: add V&V report for ADF PR gate reconciliation
Verification and validation report for Gitea #1122. All 633 tests passing, 0 clippy warnings, 0 production-critical UBS findings. 8 acceptance scenarios verified. 3 documented follow-ups (non-blocking). Refs terraphim/terraphim-ai#1122
