- version: bump to 4.0.0; release CI cross-checks the test literal
- package-smoke: use Address{} (was ambiguous Address{0})
- install prefix: suppress the SafetyHook install leak; gate DirectXMath re-export; add a hygiene check
- backend: ratify input_intercept as a second confined island + source-level gate
- MSVC debug: pin _ITERATOR_DEBUG_LEVEL=0 so the OOM-injection tests pass under the debug STL
