docs: add refactor plan for secret type introduction
- Add comprehensive refactor plan with 10 proposals across 4 phases - Include secret locations inventory with tracking tables - Document 16 secret fields across Domain, Application, Infrastructure, and Testing layers - Add entry to active refactorings index
