fix(safety): never put str into FilterResult.error; untrack fixtures

Critical: keep rsp.error=None on deny so run_filters returns structured dict instead of raising TypeError. Add full-chain integration tests. Also untrack generated report/audit, relax fork-bomb regex, tighten nested $() detection, infer code-block language, avoid mutating policy. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
