Support multiple vLLM ingress hosts

Adds vllm.ingress.hosts as a list-based configuration for the optional vLLM ingress while keeping vllm.ingress.host as the backwards-compatible fallback.

This lets the runtime Helm release explicitly own llm.underpassai.com and vllm.underpassai.com while releasing 0.5b.llm.underpassai.com for the operator vLLM release.
