Fix Gradio 5.x Chatbot format error and pin version

Add type="tuples" to gr.Chatbot() components so Gradio 5.x
accepts the tuples-format history the app produces. Pin
gradio>=5.0,<6.0 in requirements.txt to prevent future breakage.
