Merge origin/feat/adaptive-thinking: keep audited adaptive-thinking work

The local commit supersedes the pushed one (same feature + post-review audit
fixes: budget-path effort clamp, real anthropic orchestration test, extra
coverage, comment fixes). Tree is taken entirely from the local side.
