Expected behavior

WPCOM sites should display with the following format in the WP Mobile App Blog Info field:

<Type: (wpcom) REST URL: [SITE_URL] Plan: Free (1)><Unlocalized Plan: Free (1)>

Actual behavior

WPCOM sites listed as self-hosted sites instead:

<Type: (self_hosted) Self-hosted URL: [SITE_URL]>

Also, some of those sites are listed multiple times.
