Update external links

- xcp-rrdd moved into main xen-api repo
- Linux lxr has moved
- qemu filename changed, use lxr as well for convenience
- freebsd is now also available on lxr
- tapdisk3 blog post is now only on archive.org
- link tapdisk3 primarily to the blktap github repo
